Here is the "template" of sorts that I am using to list polls, just so new additions can be uniform with the page's content.

Poll title (hyperlinked) and then depending on the amount of information presented in the headline, I either add a summary of the information or include the subtext. If summing up the information in your own words, use (parentheses). Subtext should be unhyperlinked, but otherwise untouched. (If including subtext, it is recommended that you end with a period to help separate the information that follows.) Then organization that conducted the poll and date.
